New Patient Appointment Policy

To help us provide timely and efficient care, all new patients are required to complete their new patient paperwork PRIOR to their scheduled appointment.

If you are unable to complete the paperwork online, please arrive at least 20 minutes before your scheduled appointment time to complete the required forms in the office. Patients who have not completed their paperwork and do not arrive 20 minutes early may need to reschedule their appointment.

Cancellation & No-Show Policy

We understand that unexpected circumstances happen. However, patients who miss a scheduled appointment without calling or providing notice will be subject to a $75 no-show fee.
